2010 BENEFIT CHOICES
Medical Insurance Options*
- ELIGIBILITY: Full-time employees routinely scheduled for at least 60 work hours biweekly are eligible for the employer provided benefit. Employees routinely scheduled for at least 30 hours, but not more than 59 hours biweekly (part-time) may purchase this benefit with pre-tax dollars.
- WHO PAYS: Liberty Hospital pays the majority of the cost for full time employees. You may select from the following coverage options:

- WHAT YOU RECEIVE: You may choose from three different plans. There are two PPO plans that allow you to select your physician from a list of network providers. There is also a managed care plan that affords members the flexibility to access care directly from providers in the network. There are no referral requirements. All plans offer a prescription drug card.
Dental Insurance Options*
- ELIGIBILITY: Employees routinely scheduled for 30 hours or more biweekly.
- WHO PAYS: Liberty Hospital pays a portion of the cost. You may select from the following coverage options:

- Family coverage
- WHAT YOU RECEIVE: You may choose from three different plans. One plan offers an orthodontic benefit. All plans cover 100% of the cost of routine dental maintenance. Deductible and coinsurance apply to restorative dental services.
Paid Days Off (PDO)
- ELIGIBILITY: Full and part-time employees are eligible to use accumulated hours after 90 days of employment.
- WHO PAYS: Liberty Hospital pays the full cost of this benefit.
- WHAT YOU RECEIVE: You may take time away from work for personal business, illness, holidays and vacation. Full-time employees (80 hours biweekly) accrue 23½ -8 hour shifts in a year. Accrual rates increase with length of service. Part-time employees earn prorated hours for each hour worked. A person working half-time (40 hour biweekly) will accrue approximately three work weeks of time off in a year.
Extended Illness Bank (EIB)
- ELIGIBILITY: Full-time employees routinely scheduled at least 60 hours biweekly.
- WHO PAYS: Liberty Hospital pays 100% of the cost of this benefit.
- WHAT YOU RECEIVE: Paid time off for illnesses that exceed 48 scheduled hours in duration. Shorter absences due to illness are covered by the Paid Days Off benefit (see above). Full-time employees (80 hours biweekly) accrue 48 hours of EIB each year.
Flexible Spending Accounts*
- ELIGIBILITY: Employees routinely scheduled 30 hours or more biweekly.
- WHO PAYS: You may defer pre-tax earnings to pay for eligible medical expenses or work-related dependent care expenses.
- WHAT YOU RECEIVE: The ability to utilize pre-tax earnings to purchase eligible goods and services.
Term Life Insurance and Accidental Death & Dismemberment Insurance*
- ELIGIBILITY: Full-time employees routinely scheduled at least 60 hours biweekly.
- WHO PAYS: Liberty Hospital pays 100% of the cost of the base benefit. Employees may purchase additional coverage at competitive group rates.
- WHAT YOU RECEIVE: A $50,000 term life insurance benefit which pays $100,000 in the event of an accidental death. You may purchase an additional $50,000, $100,000, $150,000 or $200,000 of term life coverage.
Vision Care Plan*
- ELIGIBILITY: Employees routinely scheduled 30 hours or more biweekly.
- WHO PAYS: You pay the premium cost through pre-tax payroll deduction.
- WHAT YOU RECEIVE: Provides coverage for eye exams and the purchase of eye wear for you and your family.
Accident Insurance
- ELIGIBILITY: Employees routinely scheduled 30 hours or more biweekly.
- WHO PAYS: You pay the biweekly premium cost.
- WHAT YOU RECEIVE: Pays you cash to help with out-of-pocket costs due to an accidental injury.
Term Life Insurance for Spouse and Children*
- ELIGIBILITY: Full-time employees routinely scheduled at least 60 hours biweekly.
- WHO PAYS: Liberty Hospital pays 100% of the cost of the base benefit. Employees may purchase additional coverage at competitive group rates.
- WHAT YOU RECEIVE: A $2,000 term life insurance benefit on your spouse. You may purchase an additional $10,000, $15,000 or $25,000 policy. A $1,000 term life insurance benefit on each eligible child. You may purchase an additional $5,000 or $10,000 benefit.
Long Term Disability*
- ELIGIBILITY: Employees routinely scheduled for at least 60 hours biweekly.
- WHO PAYS: Liberty Hospital pays 100% of the cost of the base benefit. Employees may purchase additional coverage.
- WHAT YOU RECEIVE: Income protection in the event you are disabled. The base benefit provides a monthly benefit of 50% of wages to a maximum of $1,000 per month. Employees may pay extra for a 60% monthly benefit with a $7,500 monthly maximum. Benefit payments commence the 91st day after the disabling injury/illness.
Retirement Plan
- ELIGIBILITY: Employees who have completed a year of service that includes at least 832 hours (16 hours per week).
- WHO PAYS: Liberty Hospital pays 100% of the cost.
- WHAT YOU RECEIVE: An employer contribution currently equivalent to 6% of your annual taxable earnings. Those contributions are fully vested after three Years of Service. You may also contribute to the plan. Your constributions are always fully vested.
Educational Assistance
- ELIGIBILITY: Full and part-time employees.
- WHO PAYS: Liberty Hospital pays 75% of eligible expenses at enrollment/registration and reimburses 25% upon completion.
- WHAT YOU RECEIVE: Payment for undergraduate or graduate coursework. Also covers some types of continuing education and certification expense.
Supplemental Life Insurance*
- ELIGIBILITY: Employees routinely scheduled at least 30 hours biweekly.
- WHO PAYS: You pay 100% of the cost.
- WHAT YOU RECEIVE: Individually issued whole life insurance policies. Coverage is available for you, your spouse or your children/grandchildren.
U.S. Savings Bonds
- ELIGIBILITY: All employees
- WHO PAYS: You pay 100% of the cost.
- WHAT YOU RECEIVE: Bonds may be purchased through convenient payroll deduction.
Employee Assistance Program (EAP)
- ELIGIBILITY: All employees
- WHO PAYS: Liberty Hospital pays 100% of the cost.
- WHAT YOU RECEIVE: The EAP offers employees and their immediate family members confidential, short term counseling services for personal problems.
Direct Deposit
- ELIGIBILITY: All employees
- WHO PAYS: Liberty Hospital pays 100% of the cost.
- WHAT YOU RECEIVE: Biweekly, on Friday morning, your paycheck is transferred electronically to your bank.
Credit Union
- ELIGIBILITY: All employees
- WHO PAYS: No cost
- WHAT YOU RECEIVE: Payroll deduction and automatic deposits can be made to one of two available credit unions.
Employee Discounts
- ELIGIBILITY: All employees
- WHO PAYS: You pay 100% of the cost.
- WHAT YOU RECEIVE: You may purchase discounted tickets to area attractions. Food items purchased in the cafeteria are also discounted.
Jury Duty
- ELIGIBILITY: All employees
- WHO PAYS: Liberty Hospital pays 100% of the cost.
- WHAT YOU RECEIVE: Your regular wages for hours missed due to jury duty.
Leave of Absence (LOA)
- ELIGIBILITY: Varies, depending on type of leave.
- WHO PAYS: Liberty Hospital pays 100% of the cost of PDO or EIB hours that may be allocated to employees on LOA.

Workers’ Compensation
- ELIGIBILITY: All employees
- WHO PAYS: Liberty Hospital pays 100% of the cost.
- WHAT YOU RECEIVE: Payment up to 66 2/3% of gross pay for work time lost due to work related injury/illness.
