What to Bring
The Necessities
We ask that you bring only essential toiletries, pajamas or nightgowns, robe and non-skid slippers. If you have jewelry, a large amount of cash or credit cards with you, we suggest you send these items home or ask to have them stored in the hospital’s safe. Dentures and eyeglasses should be kept in protective containers when not in use. The hospital cannot be responsible for the loss or damage of such items.

Your Personal Health Record
One of the best ways you can help us provide the safest care possible is to keep a personal health record with you that contains basic information such as: 
  • A complete list of all your medications and the dosage 
  • Your immunization record 
  • A list of any allergies you may have 
  • Health conditions

Often, it is hard to track down this information after you are hospitalized; yet this information is critical to know as we develop your treatment plan.

Liberty Hospital is pleased to provide you with a simple form that you can print out and complete prior to your arrival. We recommend completing this form in pencil and carrying it with you at all times, making updates as needed.
