The Necessities
We ask that you bring only essential toiletries, pajamas or nightgowns, robe and non-skid slippers.

We advise you to leave all valuable personal property at home. This includes (but is not limited to):
      1. Clothing
      2. Money
      3. Credit cards
      4. Jewelry
      5. Electronic devices (computers, iPads, iPods, cell phones, 
          portable DVD players, etc.)

If you come to the hospital with these items, we advise you to send them home with a family member or another trusted representative. We are not responsible for them.

We understand that you may need certain valuable personal items, such as dentures, glasses, hearing aids, etc., during your stay with us. If you choose to bring these items to the hospital with you, you or your patient representative is responsible for the safekeeping of these items and the hospital cannot be held responsible for them. Your caregiver can provide you with a special container to keep them in upon request.

If you cannot send your valuable personal items home, you may store them in the hospital's vault. Please ask your caregiver about this service.

Your Personal Health Record
One of the best ways you can help us provide the safest care possible is to keep a personal health record with you that contains basic information such as: 

      1. A complete list of all your medications and the dosage 
      2. Your immunization record  
      3. A list of any allergies you may have
      4. Health conditions

Often, it is hard to track down this information after you are hospitalized; yet this information is critical to know as we develop your treatment plan.

Liberty Hospital is pleased to provide you with a simple form that you can print out and complete prior to your arrival. We recommend completing this form in pencil and carrying it with you at all times, making updates as needed.
