Hospital 2012 Diaper Giveaway Complete Rules & Regulations
The Liberty Hospital 2012 Diaper Giveaway is open to all Kansas
and Missouri residents. Void where prohibited by law.
How it Works Liberty Hospital will giveaway a year’s supply of diapers
(or up to $800). Contestants must register by signing up online at www.libertyhospital.org/baby. One winner will be selected. Contest period ends at
11:59 p.m. on February 29, 2012. The winner will be selected randomly on March
1, 2012 from among all eligible entries received, and notified by the close of
business on March 2, 2012.
Eligibility
Contest is offered only to legal U.S. residents living in
Kansas or Missouri. Entrants must be 18 years of age or older at the
time of entry. Employees of Liberty Hospital, affiliated companies, contest
co-sponsors, and their advertising or promotional agencies (collectively, the “Contest
Entities”), and the immediate families or members of the same households of
such individuals, are not eligible. Proof of age, identity, and eligibility
must be furnished upon request. All entry form information must be complete and
accurate. Sponsor will not award a prize to anyone not meeting eligibility
requirements at the time of entry. Sponsor reserve the right to disqualify any
entrant if these contest rules are not followed.
Prize
The prize, provided by Liberty Hospital is one American Express
prepaid gift card for the amount of $800, equivalent to a year’s supply of
diapers. It is required that the winner follows the guidelines of American
Express to use the prize. The prize cannot be redeemed for cash or substituted
for any other items by winner. The winner cannot assign or transfer prize. The
winner will be selected randomly from among all eligible entries received and
notified at the email address originally submitted. Odds of winning depend on
the number of eligible entries received.
Limitations of Liability
Liberty Hospital and its agencies are not responsible for any errors, inaccuracies, or omissions on its website and collateral
material related to this promotion and assumes no liability or responsibility
for any reason that the website is unavailable or working incorrectly during
the contest period. Due to Internet email blocking services, mechanisms and
filters, Liberty Hospital cannot be responsible for deleted or undelivered
contest winner notification email that does not reach the intended recipient.
The recipient of the Liberty Hospital 2012 Diaper
Giveaway will be notified by March 2,
2012 by e-mail including complete instructions on how to redeem their prize.
The prize recipient must respond to the contest notification email within 72
hours, or otherwise forfeit the prize. Except where prohibited by law, recipient
agrees to the publication of their name for online posting, and/or any
advertising and publicity without further compensation. For name of contest
winner, mail a self-addressed stamped envelope to: Liberty Hospital 2012 Diaper Giveaway, c/o Matt
Smithmier/Public Relations, 2525 Glenn Hendren Drive, Liberty, Missouri 64068.
The offer is void where prohibited by law or otherwise
restricted. The Liberty Hospital 2012 Diaper Giveaway may be discontinued by
the sponsor at any time. Sponsor is not responsible for lost, late, or
misdirected entries. All federal, state and local taxes, insurance, licensing,
registration and title fees for contest are the sole responsibility of the
participants. By participating in this offer, entrants agree to be bound by
these rules and the decisions of Liberty Hospital that are final.
