Eligibility
Full-time employees routinely scheduled for at least 60 work hours biweekly are eligible for the employer-provided benefit. Employees routinely scheduled for at least 30 hours, but not more than 59 hours biweekly (part-time), may purchase this benefit with pretax dollars.
Who pays
Liberty Hospital pays the majority of the cost for full-time employees. You may select from the following coverage options:
- Single coverage.
- Employee and spouse.
- Employee and children.
- Family coverage.
What you receive
You may choose from two different plans. These are PPO plans that allow you to select your physician from a list of network providers. There are no referral requirements. Both plans offer a prescription drug card.

What you receive
You may choose from two different plans. One plan offers an orthodontic benefit. All plans cover 100 percent of the cost of routine dental maintenance. Deductible and co-insurance apply to restorative dental services.

What you receive
A $50,000 term life insurance benefit which pays $100,000 in the event of an accidental death. You may purchase an additional $50,000, $100,000, $150,000 or $200,000 of term life coverage.

Liberty Hospital pays 100 percent of the cost of the base benefit. Employees may purchase additional coverage at competitive group rates.
What you receive
A $2,000 term life insurance benefit on your spouse. You may purchase an additional $10,000, $15,000 or $25,000 policy. A $1,000 term life insurance benefit on each eligible child. You may purchase an additional $5,000 or $10,000 benefit.

What you receive
Income protection in the event you are disabled. The base benefit provides a monthly benefit of 50 percent of wages to a maximum of $1,000 per month. Employees may pay extra for a 60 percent monthly benefit with a $7,500 monthly maximum. Benefit payments commence the 91st day after the disabling injury/illness.

Lump sum benefit upon initial diagnosis of a covered illness. Benefits may be used for any purpose chosen, such as to help pay for the nonmedical costs associated with catastrophic illnesses: lost wages, rehabilitation, family care and transportation.

What you receive
Individually issued term life insurance policies. Coverage is available for you, your spouse or your children.

Employees who have completed a year of service that includes at least 1,000 hours (20 hours per week).
What you receive
Liberty Hospital's contributions are made, primarily in the form of a matching contribution. In order to receive matching contributions, you must contribute to the retirement plan. Those contributions are fully vested after three years of service. Your contributions are always fully vested.

Full- and part-time employees are eligible to use accumulated hours after 90 days of employment.

Liberty Hospital pays the full cost of this benefit.
What you receive
You may take time away from work for personal business, illness, holidays and vacation. Full-time employees (80 hours biweekly) accrue 23.5 8-hour shifts in a year. Accrual rates increase with length of service. Part-time employees earn prorated hours for each hour worked. A person working half-time (40 hours biweekly) will accrue approximately three work weeks of time off in a year.

What you receive
Paid time off for illnesses that exceed 36 scheduled hours in duration. Shorter absences due to illness are covered by the PDO benefit (see above). Full-time employees (80 hours biweekly) accrue 48 hours of EIB each year.

Liberty Hospital pays 75 percent of eligible expenses at enrollment/registration and reimburses 25 percent upon completion.

Payment for undergraduate or graduate course work. Also covers some types of continuing education and certification expense.
