How to request copies of your medical records
We offer three ways for you to access your medical records:
- Free online access through the hospital or clinic patient portals. The best way to access your health records is via our patient portals, where you can see much of your hospital information anytime it's convenient for you. If you aren't signed up, ask your care team to help you sign up the next time you visit your clinic or for the hospital portal, click here to sign up.
- Electronic delivery If you are a patient or guardian, you can request to have your medical records sent to you in an electronic format by completing this online form.
- Hard Copies If you are a patient or guardian and need hard copies of your medical records for yourself or a third party: Download, print and complete the Paper Authorization for Release Form and then mail, fax or deliver in person to Liberty Hospital address or fax number below. You can also call us at 792.7067, and we can mail you the form.
We allow limited in-person pickup for urgent needs. Call 816.792.7067 in advance to discuss and schedule an appointment.
If records are needed for treatment or for an appointment within the next 48 – 72 hours, physician offices or hospitals can request records be faxed to them at no additional charge to the patient.
