Patient rights and responsibilities

Our vision is to be the leader in providing high-quality, patient-centered care. As a patient or patient representative, you can expect:

  1. A healing environment that preserves dignity and contributes to a positive self-image.
  2. To be safe from neglect and exploitation, as well as verbal, mental, physical or sexual abuse.
  3. A safe and secure environment. You will have access to protective services while in the hospital, and you can consult with the appropriate protective agency prior to discharge.
  4. Impartial access to treatment or accommodations regardless of race, color, creed, sex, sexual orientation, national origin, religion, age, gender identity, communicable disease, disability or source of payment.
  5. The right to choose who may visit you during the visitation hours established by the hospital.
  6. To have all cultural, psychosocial, spiritual and personal values, beliefs, and preferences respected and supported.
  7. Personal privacy—to have all communication and records concerning your care kept confidential.
  8. To know the identity and professional status of your caregivers.
  9. Supportive care, including appropriate management of pain and treatment of uncomfortable symptoms.
  10. To appoint a person to make healthcare decisions on your behalf, to the extent permitted by law, in the event you lose the capacity to do so.
  11. To make an advance treatment directive and have it honored.
  12. To be involved in your care. To be informed on the hospital's plan of care for you. We will provide adequate information and answer any questions or concerns regarding your care.
  13. To participate in discharge planning, including being informed of service options that are available to you and given a choice of agencies that provide the service.
  14. To accept or refuse any procedure, drug or treatment and to be informed of the possible consequences of any decision.
  15. To consent or refuse care that involves research, experimental treatments or educational projects, clinical trials, and that refusal to participate or discontinuing participation will not compromise access to care.
  16. To receive information in a manner you can understand.
  17. To give or withhold informed consent to produce or use recordings, films or other images for purposes other than your care.
  18. To review your medical record and obtain copies of the record at a reasonable fee.
  19. To have your personal possessions that you brought to the hospital reasonably protected.
  20. To have complaints reviewed by the hospital; be informed of the hospital's patient complaint/grievance procedures, including a timely written notice of the resolution.
  21. To be informed of these rights.

Patient and Visitor Responsibilities

In as much as patients have rights here at Liberty Hospital, patients and their visitors also have responsibilities to our staff and facilities. As a patient or visitor at Liberty Hospital, you also have the following responsibilities:

  • Liberty Hospital and its Affiliates complies with applicable State and Federal civil rights laws and does not tolerate discrimination against any person, including patients and health care team members and staff on the basis of race, color, national origin, ethnicity, culture, disability, age, sex, religion, socioeconomic status, sexual orientation, gender identity or expression and source of payment.
  • To Follow Liberty Hospital’s rules and regulations affecting patient care and conduct.
  • To be considerate of Liberty Hospital’s facilities and equipment.
  • To comply with Liberty Hospitals policies.
  • To respect the rights of Liberty Hospital employees and other personnel. Just as you would expect courteous treatment, so do the staff, visitors and personnel of Liberty Hospital. You have the responsibility to treat all staff, visitors and personnel with the same courtesy and respect you expect. Staff members reserve the right and are encouraged to file personal actions against any patient or visitor who rises to the level of criminal assault and battery in accordance with Missouri State law.
  • To report, to the best of your knowledge, accurate and complete information regarding any matters pertaining to your health to the physicians and other healthcare professionals caring for you.
  • To be informed and to ask questions by making an effort to understand your healthcare provider for information relating to y our treatment. Your healthcare provider may not know when you’re confused or uncertain, or just want more information. If you don’t understand the medical words they use, ask for a simpler explanation.
  • To follow the treatment plan recommended by the healthcare provider responsible for your care. This includes following the instructions of the other health team members such as nurses and physical therapists, as they carry out the coordinated plan of care. It is your responsibility to tell your healthcare provider whether or not you can and want to follow the treatment plan recommended for you. The most effective plan is the one that all participants agree is the best and is carried out exactly.
  • To understand how to continue your care after you leave Liberty Hospital, including when and where to get further treatment and what you need to do at home to help with your treatment.
  • To accept the consequences of your own decisions and actions. If you choose to refuse treatment or not comply with the instructions given by your healthcare provider.
  • To keep appointments and cooperate with your healthcare provider. If you need to cancel an appointment, you should do so at least 24 hours before your appointment time.
  • To pay bills promptly to assure that your financial obligations for your healthcare are fulfilled. You are responsible for working with your account representative to make payment arrangements and providing the information necessary to determine how your hospital bill will be paid.
  • To know your insurance coverage and network, and to voice any request to transfer due to coverage issues, concerning your insurance plan and network. Liberty Hospital makes all attempts to assist you in knowing this information; however, the choice of which hospital you decide to use for treatment of non-emergency care is yours.
